IOGP Published Lagging and leading indicators KPI to strengthen risk control (barries) in order to prevent major incidents and also this articels describe how managing major incident risks and the practical implementation of a KPI System. 

Major incidents rarely result from a single cause but rather by multiple failures that coincide and collectivelly result in an exceptional event with severe consequences. Hazards are contained by multiple protective barrier or risk control system. this article show illustrated "swiss cheese model" from James Reason & similiar principles such as the Bow Tie & LOPA.
